Thanks to a generous gift from the Ligums family, Texas Children’s Hospital and Baylor College of Medicine offer a one-year grant to fund transition medicine projects that will:
Focus on quality improvement, incorporating true process and outcome metrics.
Target late adolescents and young adults who:
Have chronic conditions originating in childhood; and
Are transitioning from pediatric to adult care.
Include input and feedback from patients and families.
Demonstrate partnerships with appropriate organizational leaders and/or interest groups.
The grant will be offered annually and open to all healthcare providers.

The Ligums Family Transition Medicine Scholar program is designed to celebrate healthcare providers in the community who are making a positive impact in transition medicine. Nominees for the scholarship must show a dedication to either:
Transitioning patients from pediatric to adult healthcare in the community
OR
Increasing access for adults with special healthcare needs
