In 2004, the transition team at Texas Children’s Hospital launched the Transition Medicine Program to develop evidence-based transition protocols for a variety of diagnoses.

Ensuring children have safe and smooth transitions into adult care is a systematic evolution in the world of pediatrics. One that will help empower our patients and prevent urgent medical emergencies. This new standard of care is particularly important for patients with chronic or medically complex conditions.

Nearly 90% of children born today with a chronic or disabling condition will live more than 20 years. However, the health of these individuals can deteriorate if there is not a smooth transition from pediatric to adult health care. 

By utilizing the current resources and strategically investing in expanding our capabilities, we can create a system-wide healthcare transition program for all youth and their families.
