In 2004, Texas Children's Hospital established a Transition Committee with the purpose of improving the health care transition (HCT) of youth and young adults with chronic illness and disabilities from pediatric to adult-based care.

For decades, we’ve helped children born with congenital heart disease (CHD) live longer, healthier lives by revolutionizing the way these conditions are diagnosed and treated. Today, there are more adults living with CHD than children and they all need the same level of specialized care that they received during childhood.

To address the medical, educational and psychosocial needs of young adults with congenital heart disease (CHD) in order to prepare them for eventual, informed, timely and uninterrupted transition and transfer to adult congenital heart disease care.

The Texas Children's Diabetes and Endocrine Care Center enjoys caring for teens with diabetes and endocrine conditions.
