Refer a Patient Metabolic Health and Bariatric Surgery Center

To refer your pediatric patient to the Metabolic Health and Bariatric Surgery Center, please complete the following steps.

General Information

• Patients must be between the ages of 12-20 years to qualify for care with the Metabolic Health and Bariatric Surgery Center
• Patients must have a BMI of 35 or greater
• Patients are not eligible for Metabolic Health and Bariatric Surgery services if there is a history of bipolar disorder or schizophrenia

1. Review Guidelines
   • Texas Children’s Hospital North Austin offers bariatric surgery, only (no medication management)
   • Texas Children’s Hospital The Woodlands offers comprehensive services inclusive of medication management and bariatric surgery
   • To refer for Medication Management
     • Patient must be at least 12 years of age and have a BMI of 35 or greater
   • To refer for Bariatric Surgery
     • Age requirement
       • Females must be 14 years of age or older
       • Males must be 15 years of age or older
   • Private Insurance Requirement
     • BMI of 35 or greater with at least 1 obesity related comorbidity
   • Medicaid Requirement
     • BMI of 40 or greater
