Nursing Professional Development Graduate and Post-Graduate Nursing Students (non-APRN MSN and Doctorate)

Items below must be received no later than 60 days prior to first day:

• Written notification of your intent to start clinical
• Intake information - see attached e-form “Intake Data Form”
• Complete the e-forms and return as indicated
  • Intake Data Collection e-Form
  • Intake Data e-Form (Research)
• After you receive a confirmation of placement – complete the online orientation packet, return the signed confidentiality agreement and alternative orientation checklist via pdf format to the Office of Student Affiliations: studentaffiliations@texaschildrens.org
