upLIFT Teen
Refer a patient or enroll yourself in upLIFT
The process to begin participating in upLIFT involves three steps:
To join the upLIFT or upLIFT Teen program, an individual can either refer themselves or be referred by someone else through a specific online referral link.
If referred by someone else:
- The person making the referral will use the link to provide their own contact information (to receive updates on the referral) and the individual's contact information, including their address and county.
- The referral form will also ask for the reason for the referral, the individual's age, the birthdate of their youngest child, and a brief explanation of why they are being referred to upLIFT.
If the individual is self-referring (for upLIFT or upLIFT Teen):
- The individual will use the referral link to provide their contact information.
- They will also need to specify the reason they are seeking support from upLIFT, their age, and the birthdate of their youngest child.
For upLIFT Group:
- The individual must refer themselves using a dedicated referral link.
- They will provide their contact information, county of residence, preferred language, and indicate how they heard about the program.
What happens after submitting the online referral (for all programs):
- Phone contact: After submitting the online referral, the individual will receive a phone call from the upLIFT team.
- Eligibility and interest: During this call, the team will determine the individual's eligibility for the program and assess their interest in participating.
- Mental health screening: A mental health screener will be administered during the call.
- Program fit: The team will also assess if the program is a good fit for the individual's needs.
- Enrollment (if eligible and interested): If the individual meets the eligibility requirements and is interested in participating, they will proceed to complete enrollment forms.
- Enrollment Forms: This includes signing a consent form and completing pre-questionnaire surveys.
Once you are enrolled into the upLIFT program, the licenses social worker assigned to work with you will call you to set-up your first session.

- Maternal Mental Health Alliance (UK): While based in the UK, this website offers valuable information, real-life stories, and signposting to support for maternal mental health.
- Maternal Mental Health Leadership Alliance (MMHLA): This organization focuses on advocating for policies, building partnerships, and curating information to improve maternal mental health. It offers fact sheets and resources.
- Maternal Mental Health Now: This California-based organization offers resources, information, and support for parents and families affected by PMADs.
- National Maternal Mental Health Hotline: This hotline offers 24/7 free, confidential support via call or text: 1-833-TLC-MAMA (1-833-852-6262). It can provide emotional support, information, and referrals.
- Perinatal Mood and Anxiety Disorders – CAMH: This Canadian website offers information for both professionals and the public about PMADs, including symptoms and treatment options.
- Perinatal Mood Disorders - UNC School of Medicine: This resource provides detailed information about depression and anxiety during and after pregnancy, including symptoms and related conditions.
