Congratulations on your affiliation assignment to Texas Children's Hospital with the Department of Physical Medicine and Rehabilitation. We look forward to meeting and working with you. This site has been designed to provide you with the information and resources that you will need to start your assignment with Texas Children's.

Before You Arrive

Please complete and submit the following forms one month prior to arrival.

You must submit all documents one month in advance from the following sections to therapystudents@texaschildrens.org or fax 832-825-0128.

Required Documents

  • Copy of your driver's license
  • Copy of your current American Heart Association Health Care Provider CPR card. Your CPR certification must be valid through the last day of your affiliation.
  • Copy of a completed background check

Orientation For Students and Faculty

Texas Children's Hospital has implemented an online orientation.

The instructions for access are as follows:

Each faculty and student will choose the appropriate orientation and complete only the orientation that corresponds to their role. Every student and faculty member must complete orientation and privacy training prior to entering Texas Children’s Hospital.

General Faculty and Student orientation

General Faculty and Student orientation is for those students coming to Texas Children’s Hospital that are not nursing students.

Nursing Faculty and Student orientation

Nursing Faculty and Student orientation is for any nursing student coming to Texas Children’s Hospital.

Once you have completed the orientation presentation, please sign the following document to acknowledge completion.

General Information

The Physical Medicine and Rehabilitation Department

The PM&R Department is located on the 21st floor of the West Tower of Texas Children’s Hospital.  Only the express elevators, the farthest set of elevators in the West Tower’s lobby area, access the 21st floor.  Our reception area is easily seen when you step off the elevator on the 21st floor.

The PM&R Department consists of Occupational Therapists, Physical Therapists, Physical Therapy Assistants, Speech Pathologists, Technicians, and Support Staff.  Our total staff is approximately 80 employees and is organized into two primary work teams: Inpatient Therapy and Outpatient Therapy.
