Referring Provider Instructions
To refer your pediatric patient to the Motion Analysis and Human Performance Laboratory, please select the reason for referral and complete the following steps.
- Review referral criteria
To ensure that your patient receives the best possible care, please review the referral guidelines below and submit the requested information.
Patients should meet the following criteria:- Gait impairment related to a neurological, orthopedic, or developmental disorder (for example: cerebral palsy, spina bifida, spinal cord injury, traumatic brain injury, cerebral vascular accident, genetic disorder, hereditary spastic paraparesis, idiopathic toe walking, etc.)
- Able to walk under their own power for a minimum of 10 feet without stopping. The patient must be able to walk this distance at least 10 times to complete the gait study.
- If needed, the patient may use any of the following assistive device(s): cane, crutches, standard anterior walker, or standard posterior walker. However, the gait study cannot be performed for patients who use a gait trainer or a walker with a hip belt (or pelvic support).
- For patients classified as GMFCS IV, please contact the Motion Analysis Lab at 936-267-6686 to determine whether the patient is appropriate for referral.
- Able to follow verbal commands
- At least five (5) years of chronologic age or greater, or behavior consistent with 5 years old
- Download and complete the online referral form
- Fax form to: 936-267-7904
- Provide patient with Welcome Packet
- We will submit pre-authorization to confirm insurance coverage and contact the patient’s parent/caregiver to schedule the patient’s visit at the Motion Analysis and Human Performance Laboratory.

Patients should meet the following criteria:- Age 12 and over
- Pain with throwing
- Pain with running
- Activity or sport limitations due to lower extremity pain
- Lower Extremity Injury that is not improving with traditional therapy
- Returning to sport after ACL injury
- Returning to Baseball or Softball after injury
- Returning to Dance after hip injury or surgery
- Be able to perform functional tasks like running, jumping, and squatting
- Be comfortable running on treadmill for running analysis
- Complete the online referral form
- Fax form to: 936-267-7914
- Provide patient with Welcome Packet
- We will contact the patient’s parent/caregiver to complete a phone intake and schedule the patient’s visit at the Motion Analysis and Human Performance Laboratory.
