Wheelchair Clinic

We offer a rotating clinic, twice a week, directly at the vendor site to address all durable custom equipment needs. National SeatingTM and Numotion® are the two contracted vendors offered for offsite TCH Main Campus Clinic. All other vendor appointments are held at the hospital. A prescription, signed and dated, from a physician, stating PT/OT Evaluation, patient’s date of birth, and patient’s diagnosis is required for scheduling an appointment.

All insurance (Medicaid, Medicare, and Private) require a physical or occupational therapist to perform an evaluation, an assessment, and documentation for all medically necessary durable medical custom equipment. This includes wheelchairs, strollers, bath chairs, car seats, gait trainers/walkers, and standers. It is important to have an experienced therapist present for all equipment evaluations to ensure your child receives the appropriate equipment, patient/family goals are fully addressed, and the equipment has the necessary growth. Your child is welcome to come to Wheelchair Clinic while still receiving therapy elsewhere.
