Bereavement Support Death Certificates

The funeral home will send the death certificate to your loved one’s attending doctor for signature. Once signed, the doctor will return the certificate to the funeral home, which will then file it with the Bureau of Vital Statistics. This process can take a few weeks. Fees for this service vary, and credit cards are typically accepted.

You can request copies of the death certificate by fax, mailonlineor in person. For more information or questions, you can contact the Bureau of Vital Statistics at 713-837-0311.

After death, your loved one’s MyChart account will no longer be accessible. However, you can still request medical records and autopsy results from the Health Information Management (HIM) department.
