Referring Provider Instructions
To refer your patient to the Adult Congenital Heart at Texas Children’s Heart Center, please complete the following steps.
- Complete the online referral form and upload supporting medical records and test results
If you are unable to submit supporting medical records and test results via our online form, please fax your referral order and supporting clinical information to 832-824-7333.
- Upload images to our image sharing platform
If you are referring a patient and have cardiac images you would like to share with us, upload them to our Image Sharing platform.
