Becoming a Patient
The Neuropsychology Service provides evidence-based assessment and treatment of patients with acute and chronic medical illness, as well as those without medical illness in need of brief and targeted outpatient behavioral healthcare, in a number of specific areas. Please review the guidelines, before filling out the self-referral form.
The next step is to complete the required new patient referral paperwork. You can start by visiting the Psychology Referral Page to view and complete the online or paper referral form. If you choose to use the paper referral form, please fax the completed form and required documents to 832-825-9315.
Please also include copies of the following documents when you send in the paperwork to the Referral Center:
- Divorce Decree if the biological parents of the child are legally divorced.
- Guardianship paperwork or Temporary Orders of Custody if the person seeking the evaluation and/or completing the paperwork is not the biological parent of the child.
- If applicable, Early Childhood Intervention (ECI) Evaluation, school district evaluation(s), the most recent IEP, and any private evaluation(s) performed by another provider in the community, such as a psychiatrist, neurologist, developmental pediatrician, or psychologist.
