Request for Project

Items below must be received no later than 60 days prior to the first day:

• Written notification of your intent to start
• Intake information – complete the e-forms and return as indicated
  • Intake Data Collection e-Form
  • Intake Data e-Form (Research Project)
• After you receive a confirmation of acceptance – complete the online orientation packet, return the signed confidentiality agreement and alternative orientation checklist via pdf format the Office of Student Affiliations: studentaffiliations@texaschildrens.org
