Q&A: Transitioning to adult based care

A priority at Texas Children’s Hospital is health care transition – the planned process of moving a pediatric patient into the adult health care system so their health and ability to function is optimized. As your adolescent/young adult gets ready to graduate from high school and go to college or choose some other vocational path, Texas Children’s want to make sure they are ready to take care of their health too. In the video above, I will discuss this transition to adult-based care, what age to start discussing transitioning, things your adolescent should know before transitioning to adult care and other common questions.
