Community Cares
At Texas Children’s Pediatrics we believe strongly in the medical home model. This mean families have access to primary care that is consistent, comprehensive, family-centered, coordinated, compassionate and culturally effective. A medical home can address everything from helping your child stay healthy with preventative actions, such as staying up-to-date on their vaccines, to being there when your child is sick.
It’s important that your family has a pediatrician who knows your child. By doing so, they can help address illnesses, like the common cold or the flu, and help your family monitor and follow a plan of action for chronic illnesses such as asthma and diabetes. Having a medical home with an integrated health system can also help your family by expanding the resources available to you. Your pediatrician can help your child get any additional care they may need by referring them to a specialist.
Having access to a medical home is even more important for the hundreds of families in our community who live in medically-underserved areas. Through our Community Cares Program, we provide trusted, high-quality pediatric medical services to children who otherwise would seek care from emergency rooms or possibly go without care or treatment due to low family incomes and/or lack of health insurance. Through our Community Cares Program we collaborate and work closely with many community partners to assist families with other needed health services as well.
Services at our Community Cares Program practices include:
- Acute care/illness exams
- Vaccinations and immunizations
- Preventative /well child health care
- School and sports physicals
- Hearing and vision screening
- Assistance applying for insurance programs including Medicaid, CHIP and Harris County Gold Card
- A social worker who can link families with needed medical and social services
- Health care and nutrition education for children and families
