For Dabetes Appointments: Please bring patients meter and completed log book.
Please plan to arrive for your visit at least 15 minutes before your scheduled appointment time.
Please bring a list of all current prescription and over the counter medications that your child is taking.
Please make arrangements for patient siblings. Children are not to be left unattended during the visit.
Please bring your current insurance card.
In case a co-payment is necessary, please bring a credit card, check or cash.
