Rotation Site Description
The General Medicine service is ~36 bed acute care unit that specializes in the care of patients with renal, hepatic, and gastrointestinal disorders, as well as those with general pediatric illnesses. Additionally, all kidney and liver transplant patients are cared for on this unit, both before and after transplantation occurs.


Rotation Responsibilities
Provide clinical pharmacy services to pediatric general medicine patients, which includes:

  • Understanding the basic pathophysiology of the disease states
  • Knowing the pharmacology of each drug used
  • Identifying medication-related problems
    • Medications used with no medical indication
    • Medical conditions for which there is no drug prescribed
    • Medications prescribed inappropriately for a particular medical condition
    • Incomplete immunization regimen
    • Anything inappropriate in the current medication therapy regimen (dose, dosage form, schedule, duration, route of administration, method of administration)
    • Presence of therapeutic duplication
    • Prescription of medications to which the patient is allergic
    • Presence or potential for adverse drug events
    • Presence or potential for clinically significant drug–drug, drug–disease, drug–nutrient, or drug–laboratory test interactions
    • Interference with medical therapy by social, recreational, OTC, or nontraditional drug use
    • Patient not receiving full benefit of prescribed medication therapy
    • Problems arising from financial impact of medication therapy on the patient
    • Patient or caregiver lack of understanding of their medication therapy
    • Patient or caregiver not adhering to medication regimen
  • Performing therapeutic drug monitoring
    • Aminoglycoside pharmacokinetic consultation
    • Vancomycin pharmacokinetic consultation
    • Pharmacokinetic consultation on any medications monitored by serum levels
    • Providing recommendations for total parenteral nutrition (TPN) formulations
    • Reporting adverse drug events and medication incident reports
  • Interacting effectively with members of the multidisciplinary medical team
  • Providing concise, applicable, and timely responses to requests for drug information from health care providers and patients/parents/caregivers
  • Attending lecture and teaching sessions with members of the medical team, as applicable
  • Using appropriate behavioral and cognitive techniques in interactions with the pediatric patient, parents, and/or caregivers
